You know what kind of practice you want to open. Now it’s time to plan how it will work. Phase 2 of the Serenova Method—DESIGN—maps the patient journey, staff responsibilities, scheduling, intake, billing, and technology needs into one operational blueprint, so the pieces work together before you begin building.
Phase 2 — DESIGN: Build the Operational Blueprint
You know what kind of practice you want to open. Now, how will it actually work?
Follow One Phone Call
Before the visit
- Who answers the phone?
- Once they schedule, what happens next?
- Who verifies their insurance?
- Who sends the intake forms, and how do they come back?
- What happens if prior authorization is required?
During the visit
- Where does all that information go, and who checks it is complete?
- Who collects the copay?
- How does the provider's documentation reach the people who bill for it?
After the visit
- How does the charge get to billing?
- Who follows up if the claim isn't paid?
- Who closes the loop with the patient on next steps, referrals, or a balance due?
They're not. They're your practice.
Every one of those questions is a handoff. Every handoff is a place where a patient can be delayed, a task can be dropped, or revenue can quietly slip away.
Welcome to Phase 2: DESIGN
DESIGN is where we build the operational blueprint for your practice.
In Phase 1, DISCOVER, we got clear on what you're actually building: your services, patient population, payer strategy, high-level workflows, and launch priorities. Phase 2 answers the next question: how will that practice actually run, day after day?
Before choosing systems, hiring staff, or writing dozens of policies, we need to understand how four things will move through your practice:
- Patients — from first contact to follow-up care
- Information — from intake forms to clinical notes to claims
- Responsibilities — who owns each step, and who backs them up
- Revenue — from the scheduled visit to the payment posted
The American Medical Association emphasizes structured workflows and clearly defined team responsibilities as a foundation for more efficient private practices. At Serenova, we take that idea beyond individual tasks. We look at the entire patient and operational journey, end to end, so the pieces connect before anything is built.
What We Map During DESIGN
We map twelve areas, grouped by how the work actually flows.

None of these areas works on its own. The value of DESIGN is in seeing how they connect.
Why Design Before Opening?
Because operational problems rarely stay in one department.

A gap in one place shows up somewhere else, often weeks later, and usually where it costs the most.
The technology trap
Many new practice owners choose an EHR or practice management system first, then try to build their operations around it. It feels like progress.
But without a clear workflow, you can't fully evaluate whether a system fits. You end up with workarounds, features you pay for but don't use, and staff who learn the software's habits instead of your standards.
When the workflow is designed first, technology becomes a decision you can make with confidence. You know exactly what it needs to do.
Good operations don't happen by accident. They're designed.
What You Walk Away With
By the end of DESIGN, you have an operational blueprint for the practice.
It connects five things before we begin building the infrastructure around them:
- The patient journey — every step from first contact to follow-up, mapped
- People — clear roles, ownership, and coverage for each step
- Processes — the workflows and handoffs between front office, clinical team, and billing
- Technology — the requirements your systems must meet to support those workflows
- The revenue cycle — how each visit moves from scheduled appointment to payment received
This blueprint becomes the foundation for everything in Phase 3: your systems, your SOPs, your hiring, and your training.
A quick self-check
If you're planning a practice right now, ask yourself:
- Could I sketch the path of a new patient, from first call to paid claim, on one page?
- Does every step on that path have a named owner?
- Do I know what my EHR and billing systems must do before I choose them?
- If a claim is denied or an authorization is missing, do I know who catches it?
Design It First. Build It Second.
A successful practice shouldn't depend on everyone simply "figuring it out" after opening day. Opening day should be the moment your design goes live, not the moment you start inventing it.
That's the purpose of Phase 2.

Next week: Phase 3 - BUILD. We turn the operational blueprint into working systems, processes, and infrastructure.
